Request for health reimbursement

Hello Dear, You must print and sign the form we sent you by email.

Please, the following documents must be attached to this request:

1) Original medical record 4) Doctor's prescriptions or recommendations

2) Original medical receipts 5) Original minutes kept at the scene

3) Proof of payment of invoices 6) Receipt / discharge from the hospital

You must submit the signed form and all the required documentation to the SIGAL UNIQA offices. Thank you, SIGAL UNIQA

Insurers who log in to https://clients.sigal.com.al have the opportunity to receive for the health insurance contract the status of their requests for reimbursement in real time.

At clients.sigal.com.al information is provided for:

1. Registration of your request by the company SIGAL

2. Notification for lack of documentation (with explanations)

3. Reimbursement notice (with explanations)

Insurers should keep in mind that SIGAL does not make individual communications for the above 3 points therefore tracking at clients.sigal.com.al is the only form of tracking refund status.
